Summary
The purpose of this review was to determine the validity of allegations regarding multiple care deficiencies at the Tomah VA Medical Center and William S. Middleton Memorial Veterans Hospital in Tomah and Madison, Wisconsin. The complainant alleged that a patient died as a direct result of mismanagement of his antiarrhythmic cardiac medications and that the patient was denied benefits for exposure to Agent Orange while station in Thailand during the Vietnam War. We did not substantiate most of the complainant’s specific allegations. Clinical decisions made were reasonable and justifiable. We determined that there was disparity between the complainant’s perception of the patient’s care and the actual care that was documented in the patient’s medical records. We made no recommendations.
