Healthcare Inspection Medical Device Recall Process in Veterans Health Administration Medical Centers

The purpose of this inspection was to assess medical device recall processes and specific responses to a Patient Safety Alert. We conducted unannounced inspections at 23 VA medical centers to determine if medical centers had responded appropriately. We evaluated the consistency and effectiveness of the recall process and identified best practices.
