Summary
This Office of Inspector General (OIG) Healthcare Facility Inspection program report describes the results of a focused evaluation of the care provided at the VA Philadelphia Healthcare System in Pennsylvania.
This evaluation focused on five key domains:
• Culture
• Environment of care
• Patient safety
• Integrated veteran care
• Veteran-centered safety net
The OIG made two recommendations for VA to correct identified issues in one domain:
• Environment of care
o Surveillance system
o Secure and store portable liquid nitrogen tanks
The Veterans Integrated Service Network Director and facility Director take appropriate action for the surveillance system to be fully operational.
The Director takes appropriate actions to secure portable liquid nitrogen tanks and store the tanks according to the National Fire Protection Association’s Compressed Gases and Cryogenic Fluids Code, NFPA 55 requirements.
