Combined Assessment Program Review of the Tomah VA Medical Center Tomah, Wisconsin

The purpose of the review was to evaluate selected operations, focusing on patient care administration and quality management (QM). During the review, we provided crime awareness briefings to 101 employees. This review focused on seven operational activities. The facility complied with selected standards in the following three activities: (1) medication management, (2) QM program, and (3) suicide prevention safety plans. We made recommendations for improvements in the following four activities: • Reusable medical equipment • Environment of care • Coordination of care • Physician credentialing and privileging
