Combined Assessment Program Review of the Canandaigua VA Medical Center Canandaigua, New York

The purpose of the review was to evaluate selected operations, focusing on patient care administration and quality management (QM). During the review, we provided crime awareness briefings to 221 employees. This review focused on seven operational activities. The facility complied with selected standards in the following four activities: (1) environment of care, (2) medication management, (3) physician credentialing and privileging, and (4) reusable medical equipment. The facility reported as accomplishments the community care team and the Sky Art Project. We made recommendations for improvement in the following three activities: • Suicide prevention safety plans • QM • Coordination of care
