We conducted an inquiry into various management practices and other issues at the Spark M. Matsunaga VA Medical and Regional Office Center, Honolulu, Hawaii, as a result of correspondence alleging abuse and mismanagement and a congressional inquiry from Senator Daniel K. Akaka. The Acting VISN 21 Director’s comments and implementation plans met the intent of the recommendation. We acknowledged that an independent organizational study focusing on specific leadership and management issues was needed.
